ABSTRACT

Objective: To investigate the prognostic factors related to overall survival and prognosis of patients with simple lung metastases from primary colorectal cancer. Methods: The clinical data from 90 patients with simple lung metastases from primary colorectal cancer were retrospectively reviewed. Of these 90 patients, 34 received R0 resection of lung metastases, and 56 received systemic chemotherapy. The survival analysis was performed by Kaplan-Meier method. The log-rank test was used for univariate analysis and the COX proportional hazards model for multivariate analysis of prognosis. Results: The 1-, 2- and 3-year cumulative survival rates between the patients receiving R0 resection of lung metastases and systemic chemotherapy were significantly different (97.1% vs 82.1%, P < 0.05; 88.2% vs 55.3%, P < 0.05; 74.9% vs 31.4%, P < 0.05). The 1-, 2- and 3-year disease-free survival rates of patients receiving R0 resection of lung metastases were 64.7%, 43.9% and 33.8%, respectively. The univariate analysis revealed that in patients receiving R0 resection of lung metastases, the factors of hilar or mediastinal lymph node metastases (P = 0.003), the maximum tumor size of pulmonary metastases (P = 0.007) and preoperative serum carcinoembryonic antigen level (P = 0.029) were significantly correlated with the 3-year overall survival, and the factors of lymph node metastases after curative resection of colorectal cancer (P = 0.005), the maximum diameter of pulmonary metastases more than or equal to 4 cm (P = 0.006) and preoperative serum carcinoembryonic antigen level more than or equal to 5 ng/mL (P = 0.010) were significantly correlated with the recurrence after R0 resection of lung metastases, while in patients receiving systemic chemotherapy, the factor of use of all three cytotoxic agents including fluorouracil, oxaliplatin and irinotecan (P = 0.004) was significantly correlated with the 3-year overall survival. The mulitivariate analysis revealed that in patients receiving R0 resection of lung metastases, the factor of maximum diameter of pulmonary metastases more than or equal to 4 cm (P = 0.032) was an independent prognostic factor related to the 3-year overall survival, while the factors of positive lymph node metastases after curative resection of the primary colorectal cancer (P = 0.030) and the maximum diameter of pulmonary metastases more than or equal to 4 cm (P = 0.049) were independent prognostic factors related to the recurrence after R0 resection of lung metastases. Conclusion: The overall survival of patients receiving R0 resection of lung metastases from primary colorectal cancer especially for the patients with the maximum diameter of pulmonary metastases less than 4 cm can be significantly improved. The overall survival of patients receiving systemic chemotherapy using all three cytotoxic agents including fluorouracil, oxaliplatin and irinotecan can also be significantly prolonged. ABSTRACT

Objective To evaluate the clinical value of trascranial sonography (TCS) in measuring hyperechogenic substantia nigra ( SN) area and area ratio of SN to midbrain ( S/M) for the diagnosis of Parkinson′s disease( PD).Methods A total of 109 PD patients ( PD group) and 115 normal controls (control group) underwent TCS.The area of midbrain and SN and the area ratio of S/M were measured and compared between PD group and control group .Statistical analysis of the two parameters in predicting PD was performed with receive operating characteristic ( ROC) curves.The sensitivity and specificity of each parameter and their combination were calculated .Results The hyperechogenic SN area and S/M were (0.34 ±0.27)cm 2 and (12.15 ±4.57)%in PD group,whereas (0.14 ±0.08)cm 2 and (6.37 ±3.30)%in control group respectively .The difference between the two groups was statistically significant (t=82.68, 100.83,both P7.52%,the sensitivity was 80.4%and the specificity was 78.6%.Conclusions TCS is a fast, convenient,effective and useful tool for screening PD .The combination of the SN area and S/M provided the best diagnostic parameters.

ABSTRACT

Midkine is a heparin-binding growth factor, which plays important roles in the regulation of cell growth and differentiation. The non-tagged recombinant human midkine (rhMK) is therefore required to facilitate its functional studies of this important growth factor. In the present work, rhMK was expressed in Escherichia coli (E. coli) BL21 (DE3). The expression of midkine was efficiently induced by isopropyl-beta-D-thiogalactopyranoside (IPTG). After sonication, midkine was recovered in an insoluble form, and was dissolved in guanidine hydrochloride buffer. Renaturation of the denatured protein was carried out in the defined protein refolding buffer, and the refolded protein was purified using S-Sepharose ion-exchange chromatography. The final preparation of the rhMK was greater than 98% pure as measured by sodium dodecylsulfate-polyacrylamid gel electrophoresis (SDS-PAGE) and reverse phase high performance liquid chromatography (RP-HPLC). The purified rhMK enhanced the proliferation of NIH3T3 cells.

ABSTRACT

Objective To investigate the protective mechanism of protein kinase C(PKC)and calcium sensing receptor(CaR)in ischemia preconditioned rat hearts.Methods Using cell culture method,in vitro cultured inhibitor(IPC+CaRI).Apoptosis was detected using TUNEL and Hoechst33342 cell viability was detected by MTT,the protein expression of easpase-12,calpain and CaR in endochylema were detected using Wedtetm blot.ResultsIn I/R group nucleus was shrank,big blue,chromatin concentrated,apoptotle body appeared.Other groups haddifferent fluorescence intensity varying degree,IPC+PKCI+CaRS group had more big blue nucleus.Myocardialcell viability and apoptotic rate,I/R group[(62.99±0.65)%,(19.13±0.87)%],IPC group[(78.67±0.37)%,(14.21±0.74)%],IPC+PKCI group[(71.09±0.52)%,(20.46±0.81)%],IPC+PKCI+CaRS group(66.10±0.75)%,(24.89±1.43)%],IPC+CaRS group[(69.56±0.44)%,(21.64±0.77)%],IPC+CaRI group(85.81±0.60)%,(13.12±0.69)%],all had a difference(P<0.05 or<0.01)compared with C group[(100.00)%,(6.02±0. 31)%].Western blot identified that CaR expression in IPC+PKCI and IPC+CaRS,IPC+PKCI+CaRS groupswas more than that in IPC and IPC+CaRI groups;easpase-12 had more active fragment(60×103)in I/R,IPC+CaRS,IPC+PKCI+CaRS groups;ealpain expressions in I/R,IPC,IPC+PKCI,IPC+PKCI+CaRS,IPC+CaRSgroups were higher than those in C and IPC+CaRI,I/R group was the highest one,C group the second,IPC+CaRI the third.Conclusion The interaction of PKC and CaR can reduce the intracellular Ca2+ from sarcoplasmicreticulum thus provide a protection.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficiency of micro-implant anchorage (MIA) for posterior teeth intruded and the result of the treatment of scissors bite on one-side posterior teeth.</p><p><b>METHODS</b>The study included 3 females and 1 male. All the overextruding upper posterior teeth were intruded by the MIA. The micro-implant screws were inserted into the buccal and lingual alveolar hone of the maxillary posterior teeth or the buccal alveolar hone of mandibular posterior teeth. About 0.833 N force was used to intrude the overgrowthing upper posterior teeth, and about 0.559 N force was used to draw buecally the low posterior teeth tilting lingually.</p><p><b>RESULTS</b>The overextruding upper posterior teeth were intruded 2.0 mm on average, the low posterior teeth tilting lingually were upreared buccally. All the MIA screws kept stable during the treatment, but there was a slight inflammation around the implant screws.</p><p><b>CONCLUSION</b>MIA could be used as an efficient method to correct scissors bite on one-side posterior teeth with intruding overgrowth upper posterior teeth, or uprearing buccally the tilting low posterior teeth.</p>

ABSTRACT

In order to analyze the prognosis and related factors of acute lymphoblastic leukemia (ALL), 53 newly diagnosed ALL patients were enrolled in this study. The therapeutic efficacy and prognosis of 53 cases of ALL were analyzed, the remission, relapse, overall survival and event-free survival were studied, and relation between different factors and prognosis of ALL were investigated by comparison of cases in same stage. The results showed that the complete remission was achieved in 36 out of 53 patients, the total remission rate was 67.9%, the total relapse rate was 37.7%, the median relapse duration was 6 months after remission. Median overall survival (OS) and median event-free survival (EFS) time were 4 and 1 months after remission respectively, OS and EFS rate of 18 month was 35.1% and 14.2%. The patients with different gender had significantly different EFS. Age was an independent risk factor of CR rate. White blood cell count and hemoglobin level of newly diagnosed patients were significantly correlated with OS and EFS. Absolute neutrophil count (ANC) at the end of the induction chemotherapy was an independent related factor of OS, the higher ANC, the lower risk of death. The patients with or without chemotherapy related infection had different relapse rate. The patients with bleeding after chemotherapy had lower OS when compared with those without bleeding. Serum glucose level was a significant negative prognostic factor. It is concluded that there is higher relapse rate, poor prognosis in adult ALL in comparison with children. In order to decrease the relapse rate and prolong the EFS, individual therapeutical regimens and prophylaxis of complicating diseases should be applied to ALL patients.

ABSTRACT

To analyze the prognosis and risk factors of acute nonlymphocytic leukemia (ANLL), 94 patients with acute nonlymphocytic leukemia were enrolled in this study, while survival rate and risk factors of prognosis were analyzed. The results indicated that the complete remission (CR) ratio was 51.1%. Overall survival and event-free survival rates of month 6, 12, 18, 24 were 68.6%, 55.8%, 53.8%, 46.4%, 21.3% and 57.9%, 38.6%, 33.3%, 31.6%, 0% respectively. The factors such as age<40 years, WBC<10.0x10(9)/L before chemotherapy, WBC in the period of bone marrow suppression<1.0x10(9)/L, chemotherapy within 1 month after occurrence of leukemia, blood transfusion before chemotherapy of APL had favourable influence on remission and survival rates of ANLL patients. CR1, the time to get CR, length of CR and relapse significantly correlated with prognosis (p<0.05). It is concluded that the individualized therapy concerning the risk factors should be applied to ANLL patients for improving the remission, survival rate and prognosis.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical efficacy of rectocele repair with longitudinal incision and transverse suture on the vaginal posterior wall.</p><p><b>METHODS</b>One hundred and forty-six patients with rectocele were enrolled in our study from August 1999 to August 2003. The patients were randomly divided into two groups, and received traditional repair with longitudinal incision and longitudinal suture (control group, n=74) or repair with longitudinal incision and transverse suture on the vaginal posterior wall (study group, n=72). The efficacy and complications were compared between the two groups.</p><p><b>RESULTS</b>In the study group,only one case (1.4%) had no effect, and the total effective rate was 98.7%. The mean course of treatment was (11.0+/- 1.9) days. Only two cases (2.7%) had postoperative complication. In the control group, 8 cases (11.1%) had no effect, and the total effective rate was 88.9%. The mean course of treatment was (17.4+/- 1.6) days. Twenty-nine cases (40.3%) had postoperative complications. There were significant differences in the efficacy and complications between the two groups (both P< 0.01).</p><p><b>CONCLUSION</b>The refined rectocele repair with longitudinal incision and transverse suture on the vaginal posterior wall has good efficacy with shorter curative period and less complications.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects of microwave powers on temperature changes of dental pulpal chamber in vitro.</p><p><b>METHODS</b>96 isolated teeth were prepared in three degrees of dental occlusal wear according to Smith BG's report. Temperature change values in dental pulpal chamber were recorded by thermometer (DM 6801A) while the different affect time and microwave powers acted directly and with 75% sodium fluoride glycerine paste on face of isolated teeth separately in room temperature.</p><p><b>RESULTS</b>The degree of temperature in dental pulpal chamber was going up follow the raising microwave power and prolonging time. The temperature in dental pulpal chamber with microwave acting sodium fluoride on dental occlusal was higher than acting directly dental occlusal.</p><p><b>CONCLUSION</b>The microwave power, time and degree of dental occlusal wear are very important factors on temperature change in dental pulpal chamber in our study.</p>

ABSTRACT

Objective: To evaluate the clinical value of multislice CT (MSCT) in the diagnosis and preoperative TNM staging of gastric carcinoma. Methods: Fifty patients with advanced gastric carcinoma were examined by MSCT, gastrointestinal series (GI), fiberoptic gastroscopy (FG) and transabdominal ultrasonography (US). The results of the 4 methods were compared with postoperative pathological results. Forty patients, who were diagnosed as having advanced gastric carcinoma by both MSCT and US, had their TNM staging evaluated and the results were compared with postoperative pathological TNM evaluation. Results: The detection rates of MSCT, FG, GI and US for advanced gastric carcinoma were 98%, 100%, 88% and 80%, respectively. The detection rate of MSCT was not significantly different with that of FG, but was obviously higher than that of GI (P=0.027) and US (P=0.004). The accuracy of MSCT in preoperative TNM staging was significantly higher than that of US(92.5% vs 72.5%). Conclusion: MSCT, with two-phase thin slice incremental scanning image, multiplaner reformats (MPR) and three-dimension (3D) image, is more advantaged in detecting the gross type, size, location, invasion and metastasis of advanced gastric carcinoma, thus greatly improving the detection rate and preoperative TNM staging of advanced gastric carcinoma.
